Dental Radiography Education
Dental Radiography Program Approval
We are proud to announce that our Dental Radiography Program has been officially approved by the Indiana Department of Health (IDOH).
Infuse Dental Assistant School is authorized to provide Dental Radiography education, with approval valid for a period of five (5) years. This recognition reflects our commitment to delivering high-quality training that meets state standards and prepares students for clinical success.
While the IDOH website is updated periodically, our program is currently recognized and maintained on their internal list of approved limited radiography training programs. Approval can be confirmed directly through IDOH upon request.
How to Become a Licensed Dental Radiographer
Students must follow these steps to obtain their provisional permit and dental radiographer license:
1. Enroll in an Indiana Department of Health (IDOH) approved Dental Radiographer program.

2. Complete the “Application for Provisional or Student Permit” form and submit it to IDOH. Form attached.

- The permit category is “Provisional – Dental”
- A signature from the program director/instructor of the IDOH approved program is required on the form
- Provisional Permits are valid for 6 months from the date of issue.
- Permits are to be used for the educational clinical experience. Permits cannot be used to perform x-rays unsupervised as an individual employed by a medical or dental office.
- Permits are connected with the IDOH approved program, not the student. If the student withdraws from the approved program, their permit becomes invalid.
- Permit applications may be sent in en masse via email to Radiology@health.IN.gov or via fax to 317-233-7154, Attn: Radiology. Please allow 7-10 State business days for processing
3. Successfully complete either the DANB Radiation Health & Safety exam or the DANB Certified Dental Assistant (CDA) examination.

- Students may take the DANB exam at any point during the process.
- Students will contact DANB directly to schedule and pay for the exam
- Passing the DANB exam does not automatically issue a license
- Students do not need IDOH permission or an active permit to take the exam
- If a student has passed one of the approved DANB exams in the past, they do not have to take it again
4. Once the applicant has completed the IDOH approved educational program, the program director/instructor OR the dentist who supervised the clinical portion of the program will complete the “Application for Proficiency Certification for Limited Radiographer” form and submit it to the Indiana Department of Health (IDOH) Radiology division along with a copy of the program diploma/certificate of completion.

- This form is an attestation from the instructor or dentist “vouching for” the student’s suitability for licensure.
5. Complete the “Application for Radiology License” form and mail it into the IDOH Radiology Division with a check or money order for the $60 licensing fee.

- The proficiency attestation, program completion certificate, license application, and license fee payment may be mailed in altogether in one mailing.
- Students are welcome to include their DANB certificate with their application packet, but it is not required. Passing the exam can be verified through the DANB website.
6. Once all the above is completed, the applicant will be issued an Indiana Dental Radiographer license. The license is valid for 2 years. Renewal fee is also $60.
